“Shell Brain” and Its Consequences | Tales From the Bottle

Qxir | May 15, 2026

If you’re having a scan done on your brain, you might already be in a scary situation, but it doesn’t get much scarier than the scan showing a massive hole in your grey matter.

“Hydrocephalus is a condition in which cerebrospinal fluid (CSF) builds up within and/or around the brain, which can cause pressure to increase in the skull. Symptoms may vary according to age. Headaches and double vision are common. Elderly adults with normal pressure hydrocephalus (NPH) may have poor balance, difficulty controlling urination or mental impairment. In babies, there may be a rapid increase in head size. Other symptoms may include vomiting, sleepiness, seizures, and downward pointing of the eyes.
Hydrocephalus can occur due to birth defects (primary) or can develop later in life (secondary). Hydrocephalus can be classified via mechanism into communicating, noncommunicating, ex vacuo, and normal pressure hydrocephalus. Diagnosis is made by physical examination and medical imaging, such as a CT scan.
Hydrocephalus is typically treated through surgery. One option is the placement of a shunt system. A procedure called an endoscopic third ventriculostomy has gained popularity in recent decades, and is an option in certain populations. Outcomes are variable, but many people with shunts live normal lives. However, there are many potential complications, including infection or breakage. There is a high risk of shunt failure in children especially. However, without treatment, permanent disability or death may occur.
Hydrocephalus affects about 0.1–0.6% of newborns.Rates in the developing world may be higher. Normal pressure hydrocephalus may affect up to 6% of people over 80. Description of hydrocephalus by Hippocrates dates back more than 2,000 years. The word hydrocephalus is from the Greek ὕδωρ, hydōr, meaning ‘water’ and κεφαλή, kephalē, meaning ‘head’.”

There are case days of this with children. Same condition, except it happens while they’re an infant. In many cases, these kids can go on to be extremely intelligent and capable, even genius level IQs. This is all because the brain develops with and utilizes the brain tissue it does have to its maximum potential.

75 IQ is actually pretty normal, it’s just below average. It’s also not really a useful metric, since WL was still a functional adult with a job and family. A lot of people go through life with a lower IQ and function perfectly fine. IQ only measures your ability to work around certain examples, and does correlate with how educated you are, making it a not-super-useful metric. Kind of like the mental equivalent of BMI. Good as a rule of thumb, not good as an actual measurement.

The fact his brain was compressed and able to function mostly fine makes me wonder how compressed the brain can get. If a brain was as densely packed as his but took up the space it was supposed to, would he be more intelligent due to technically having "more brain"?

I’m no neuroscientist but I’ve heard that most of the brain’s processing happens on the surface of the brain (gray matter). White matter, is responsible for connecting everything together so it kinda makes sense that he’d still be able to think and do just fine; that shell is all he needs!
