The Polio vaccine changed everything. But so did the way it was created, approved, and distributed.

I wanted to tell the story of how Dr. Albert Sabin’s oral polio vaccine changed the world, not just through science but through logistics. In this video, I show how a simple sugar cube with pink syrup became one of the most powerful public health tools ever used. I trace the creation, approval, and distribution of the Sabin oral vaccine, the rivalry with Jonas Salk’s injected vaccine, and the behind-the-scenes work of the March of Dimes that made it all possible. From iron lungs to gamma globulin, from mass field trials to the Cutter Incident, I explain how the fight against polio was as much about supply chains, funding, and coordination as it was about laboratory breakthroughs.
I explore the incredible details of vaccine logistics: tens of thousands of rhesus monkeys imported through Okatie Farms to grow poliovirus, forgotten testing grounds like Chillicothe Prison, and the massive Soviet vaccine trials that proved Sabin’s method during the Cold War. You’ll see how Morgan Island became a symbolic “monkey island,” how “Sabin Oral Sundays” immunized millions of Americans, and how cardboard cups, sugar cubes, and colored syrup became weapons against paralysis. Along the way I connect the story to culture — including how “A Spoonful of Sugar” from Mary Poppins was inspired by the oral polio vaccine — and reveal how media celebrated Jonas Salk while often overlooking Albert Sabin’s global impact.
By the end, I show how logistics, distribution, and grassroots organization crushed polio and reshaped modern vaccination campaigns. The Sabin vaccine saved millions of lives, but today in low-polio environments, the Salk vaccine is again the safer option because of vaccine-derived risks. This history matters now: eradication campaigns, global health logistics, and vaccine debates are still shaped by these lessons. If you care about public health, medical history, science, or even the cultural echoes of vaccines, this video is packed with insights. Watch as I fill 1,000 paper cups with sugar cubes to symbolize the scale of this achievement and tell the hidden story of how sugar, science, and logistics eliminated one of the world’s most feared diseases.

The problem is that logisitics is rarely glorified. Take a military, for example. The warriors on the front line get all the credit. However, it's invariably the nation with the best logistical support that wins. The front line is as good as its training. But the best training, weapons, and doctrines fail when the supply chain is severed. Hollywood just can't find a way to make an captivating thriller about wall-to-wall inventories, writing requisition chits, or how acquisition decisions are made.

Never got the oral option. I suppose by the 1970s (when I was born), polio was so eradicated that shots were sufficient?

I’m so happy videos like this are still getting made. Vaccines are truly marvels! Much appreciation to everyone who helped and continues to do so. Love the research you did. Thank you! ❤

It's been suggested that Salk was a lot better at cultivating the media than Sabin.

My grandfather suffered lifelong effects from the polio he got as a kid — it's scary that even in his lifetime, people are regressing to the point of reintroducing preventable diseases without any reason

Phil, this video is fantastic but isn't getting the reach it deserves.
Have you considered changing the title to something more provocative like "How this sugar cube saved 24,000,000 lives"?

My mom suffered polio as a 1930s child, quarantined and unable to walk for a year. Throughout her life had frequent leg cramps. I remember in the early 1960s by alphabetical order we went to downtown Berkeley heights nj to get the sugar cubes . My mom a RN was thrilll us kids were gonna be spared from getting polio.

So fun to see a shot of the studio and know where each piece came from.

Thank you for the video! That was great. I remember getting the Sabin vaccine. My father and mother took my brother and I to his hometown, I remember my cousins there also. We went to the local volunteer fire department to receive it. I really didn't know what was going on at the time, I was probably about 5 years old, but it was a part of my childhood growing up in the 60s and 70s. I really do enjoy your videos, great ideas and presentations.

15:14 I think Sabin and Salk got mixed up here? It was the Salk vaccine that had risks of vaccine-induced polio, right?

Growing up in Israel in the '90s/'2000s, we got the Sabin vaccine when we were, I believe, in 8th grade. The nurse said she'll give us to droplets on the tongue and then a hard candy. We were thrilled. Instead of a frightening injection we get a candy. Anyway, I get what I'm really trying to say is that I'm surprised than an American such as yourself haven't heard about this vaccine, but, on the other hand, it's legit: it has its advantages and disadvantages compared to the Salk vaccine. Anyway, both these men – and the people who helped them – are heroes to me, as they saved millions of lives (and quality of life) without trying to get rich from that. I wish we had more of these stories nowadays, really.

As much as i appreciate the storytelling he does and all the thing i have learned from his videos. There is no practical use for using 8000 cubes of sugar with 8000 paper cups. It'll all go to the trash. Its a nice demonstration, but that's it. Anyways, his money, his choice.

Don't worry RFK will made all on his power to defeat the eradication of Polio and Smallpox two biggest successes of World Health Organization.
A little MUCH IMPORTANT thing about Sabin vaccine: it is a live attenuated virus that reproduces on the intestinal tract: in places without basic sanitation measures like stilts the virus released in feces will "contaminate" other children vaccinating them by proxy. It is estimated a child vaccinated in those places can spread the immunization to several other children.
Sabin vaccine is a genius stuff.
Just do not tell RFK about that (he is too ignorant to even understand the concept) or he will find a way to destroy the idea even in other countries.

Really shines a light on the decay of out gov't. From helping drive solutions to solve real problems facing ordinary people to trying to undo all that progress so kids start dying or being maimed for life again.

I remember the Nova episode about both vaccines. It talked about how each vaccine worked in the body with the oral vaccine working in the gut while the injectable vaccine worked in the blood and tissues.

I remember those sugar cubes. I also remember my trumpet teacher had polio.
He could walk, but it was hard for him. He was one of the lucky polio survivors.
I also remember Thalidomide that could have been taken by my mother.
We escaped many truly terrible things.

Thanks for this Phil. With the newest climate we're going into, I'm worried that science is under attack under RFK. Wanting to ban vaccines, but then "Oh well what if we go back to a dead cell version of vaccines?" And all his other craziness. The fact we're going down this timeline and willing to let long dormant illnesses back into our lives is nuts.
I always hoped the antivaxxers would just go away, but now that stupid is the new "Smart," I fear we're in for a long, deadly road.

I am a sugar cube child. In approximately 1964 when I was about 6 years old, I remember going one day to a local elementary school where lines of children formed in front of long tables, and were given the sugar cube. I remember it being pink, but don't remember the cup or the other 2 doses. Everyone in the neighborhood went. Around the same time I remember people even came to the door collecting for the March of Dimes.
Within 2 years, we moved to Pittsburgh. There, Salk was the hometown hero and still is. There's a building at Pitt with his name! As a public health graduate I know I learned about Sabin, but until today's video I didn't remember/know he was at Cincinnati.
The generations after me have no concept of the devastating toll polio had on the nation. My mother remembers the President (FDR) had polio. There were summers where children were not permitted to go to the public swimming pools for fear of catching polio. As a young adult I knew (older) adults who had polio as a child. They walked with limps or heavy metal braces on their legs. When my children were babies 30 years ago, they were given polio vaccine as a sweet, pink liquid in a small plastic tube to suck on. How much more pleasant than an injection.
