Allow me to explain the "why are you so fat" question.
Dragons are magical creatures. Ice zombies are magical creatures. Witches giving birth to shadows are magical events. All of these things fall purely within the realm of the fantastical and the magical. I have never seen a real dragon or an Ice Zombie or a shadow crawl out of a witch's womb, and so I have no real world examples to compare them to. Thus, while I can either like them or dislike them, I must nevertheless accept them at face value.
On the other hand, I have seen obese people. I have seen what happens when people live and survive in frigid environments with a limited food source for weeks or months on end. I have seen what happens when people undertake significant treks on foot, and lug heavy objects around. So while I can suspend my disbelief of fantastical beasts because I've never encountered something in real life that thwarts that suspension, it is precisely because I have seen the effects of an extreme daily caloric deficit that Samwell Tarly remaining obese runs afoul of my ability to suspend disbelief.
